| Step 3 | Choose the period |

The PAGCOR exclusion program lets you, or a close relative on your behalf, bar one person from every gaming site and venue PAGCOR operates or regulates. You choose the period, PAGCOR enters the name in a national database, and the licensed sites read it. This page walks through the types, the periods, the forms and where each application goes.
How the PAGCOR exclusion program works, in six steps
- Choose the type. Self, family or licensee-emanating (explained below).
- Gather the documents. One valid government ID with photo and signature, plus 2x2 photos taken within 6 months.
- Choose the period. Self: 6 months, 1 year or 5 years. Family: 6 months, 1 year or 3 years.
- Fill in the form. RG Form 2 for self, RG Form 3 for family.
- Send it to PAGCOR. By the portal, by e-mail or by sealed envelope, as set out below.
- Keep your confirmation. The name enters the National Database of Restricted Persons.
PAGCOR exclusion: the three types
| Type | Who applies | Periods | Form |
|---|---|---|---|
| Self exclusion | The player | 6 months, 1 year or 5 years | RG Form 2 |
| Family exclusion | Spouse, child at least 18 years old, or parent | 6 months, 1 year or 3 years | RG Form 3 (GLDD-890) |
| Licensee-emanating exclusion | A licensed operator, on grounds such as cheating, theft or fraud | 1 year, 5 years or lifetime | RG Form 4 |
The Responsible Gaming Code of Practice, version 6.0, describes all three. Names go into the National Database of Restricted Persons, and an order applies in every PAGCOR operated and regulated gaming facility, whichever site took the application.
Self exclusion: documents, period and the first six months
For yourself, you submit one photocopy of a valid government ID with photo and signature, the fully accomplished RG Form 2 and one 2x2 photo taken within 6 months of the application date. PAGCOR’s responsible gaming page names an Online Self-Exclusion Application Portal at osea.pagcor.ph.
The first six months are irrevocable. After that window you can file a Petition to Amend the Exclusion Period (RG Form 8). When the period ends, the order lifts on its own, and you can re-apply by ticking the re-application box on the form.
Family exclusion: who can apply and what to attach
A spouse, a child at least 18 years old or a parent can apply, and the relative who needs help is the respondent on the form. The form asks for details of both people, the respondent’s government photo ID with signature and birthdate, and proof of the relationship: a birth certificate for a parent, a marriage certificate for a spouse, or the applicant’s own ID or school ID and birth certificate for a child. It also asks for two 2x2 colour photos of the respondent taken within the last 6 months, facing the camera, with no hat, glasses or mask.
If the application is not made in person, a selfie or video call is required for confirmation. PAGCOR’s responsible gaming page says to e-mail the completed application and requirements to RGD_exclusion@pagcor.ph. The Code of Practice also describes a sealed brown envelope delivered by hand or courier to PAGCOR’s Gaming Licensing and Development Department; read the current address on pagcor.ph before you send.
The form’s terms state that the excluded relative cannot collect winnings or recover losses from any gaming while the period is in effect.
How many people use it
PAGCOR publishes counts of active records in the National Database of Restricted Persons.
| Type | 2024 | 2025 | 2026, through September |
|---|---|---|---|
| Self | 246 | 2,421 | 32,890 |
| Family | 197 | 217 | 793 |
| Licensee-emanating | 26 | 38 | 117 |
| Total | 469 | 2,676 | 33,800 |
The 2026 total of 33,800 is more than twelve times the 2025 figure of 2,676. Self exclusion accounts for 32,890 of it.
What the order covers, and what it leaves to you
The order is written for PAGCOR operated and regulated gaming facilities, so the first thing to know about any site is whether PAGCOR’s records name it. The method is on the page about the PAGCOR list of licensed online casino records, and the free technical checks are on the legit checker steps. A site outside the records is outside the order, and on those you rely on your own limits.
PAGCOR also lists accredited help centres. The National Problem Gambling helpline is reached on the landline (02) 8248 9568, and the agency describes it as a 24/7 service staffed by trained para-counsellors. Time-outs, limits and other player tools are on the responsible gaming rules page, and the wider picture of which sites the order reaches is on the licensed casino basics page.
Players must be 21 or older. The same PAGCOR page lists persons under 21 and persons in the National Database of Restricted Persons among those prohibited from playing on any online gaming platform.
